Conference Overview

This program is directed toward providers working in geriatrics and long-term care and will highlight issues that impact care of seniors across multiple practice settings.

This conference will assist clinicians to improve their recognition of geriatric medical issues and to develop best practices aimed at reducing medical errors and improving consistency and quality of care.

Target Audience

The Long-Term Care Medical Community including; Medical Directors, Attending Physicians, Physician Assistants, Nurse Practitioners, and those medical professionals who are still in training.

This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of AMDA-The Society for Post-Acute and Long-Term Care Medicine and (name of nonaccredited provider). The AMDA-Society for Post-Acute and Long-Term Care Medicine is accredited by the ACCME to provide continuing medical education for physicians This live activity is approved for a maximum of _ AMA PRA Category 1 Credits ™.  Physicians should claim credit commensurate with the extent of their participation in the activity.

CMD Credit Statement:

This live activity has been pre-approved by the American Board of Post-Acute and Long-Term Care Medicine (ABPLM) for a total of  management hours and  clinical hours toward certification as a Certified Medical Director (CMD) in post-acute and long-term care medicine. The CMD program is administered by the ABPLM.  Each physician should claim only those hours of credit actually spent on the activity.
